We excel in providing outstanding patient care and customer service and we work together to make a difference in the lives of our residents, families, and staff! Responsibilities Include:
1.
Ensuring that the medically related emotional and social needs of our residents are met / maintained on an individual basis 2.
Assist with completion of assessments, interdisciplinary plans of care and discharge planning for each resident as directed.
3.
Assist with transportation needs, medical appointments, communication assistance, physical aid devices etc.
as per each resident's needs.
4.
Attend care plan meetings 5.
Conduct 1:
1 room visits with residents and provide social service counseling.
6.
Coordinate the ancillary services to meet the residents needs for dental care, podiatry, eye care, hearing services, mobility equipment, etc.
7.
Miscellaneous Filing and computer work 8.
Assist with discharge planning
Qualifications:
1.
Must have compassion for, and desire to work with, the elderly along with patience, tact and a cheerful disposition.
2.
Ability to communicate effectively with residents, families, and staff 3.
Must possess excellent oral, written, and interpersonal skills.
4.
High School graduate.
5.
SNF experience and social service certificate preferred.
6.
Bi-lingual (English and Spanish) preferred but not required.
